What happens to my Health Care benefits if I am laid off?
All coverages will cease effective the month following the month in which your layoff begins. Thereafter, if your layoff is a qualifying layoff under the Supplemental Unemployment Benefit (SUB) Plan, your Health-Care HSMDVH coverage (but not dental) will be continued by the Corporation in accordance with the following table:

After the last month for which coverage is provided by the Corporation, you may continue HealthCare HSMDVH coverage for a period of 12 additional months by paying the required monthly premium amount for such coverage.
Dental coverage continues until the end of the following the month in which your layoff began. You may continue dental coverage while on layoff by contacting Benefit Express at 1-888-456-7800 and enrolling in Cobra coverage for dental within 30 days of layoff. You may continue optional group life ins., dependent group life ins. and accidental life ins. by contacting Benefit Express at 1-888-456-7800 within 30 days of layoff
